Central California ASA Registration Requirements
The
following Items are required to
register your 'A' Travel Team:

On-line
Player/Team Registration
All players and coaches are required to register through RegisterASA.com. If you don't already have an account the Manager or Head Coach needs to
click "Free Login" to create a newaccount. Once your account
is created you will receive an e-mail with access instructions.
Login and follow the directions to create your team and add your
players/coaches. When all players/coaches are added you will see an invoice
which states your registration fees due to Central Cal ASA. Mail this
invoice with birth certificates of players requiring DOB validation
to the address on the invoice. Please
don't forget that I cannot approve your team until I receive all
funds including the $350 Metro Tournament entry fee.
When I receive your
invoice, $$$ and the required birth certificates, I will verify
age and approve the registration request. Once I approve the request RegisterASA.com will ship your new ID cards.
Additional
Named Insured Form
Insures
your play and practice facilities. A certificate will be issued
to the covered facility once the Commissioner's
office receives the
application.

Player
Medical Waiver/Affidavit/Release Form
This is
a combination form. It covers a medical waiver signed by the parent,
a player agreement signed by the player and a player release signed
by the manager(Do not turn in the "Player
Medical Waiver/Affidavit/Release Forms"), Keep this form for your records until player is
released.
Individual Player Registration Form (Winter Registration Only)
This form is used to register a player which has not registered ASA in the current year and one that wishes to participate in a Winter softball program. Please fully complete the registration form a mail to the address provided on the registration form.
New Team Registration Form (Winter Registration Only)
This form is used to register a team that was not registered ASA in the current year. Please fully complete the registration form a mail to the address provided on the registration form. |
